If you have Medi-Cal and are pregnant or a new parent, you may be eligible for Transitional Care Services (TCS). Transitional Care Services are designed to support you after discharging from the hospital. 

Call your Medi-Cal managed care plan for your more information. Their phone number is on your plan ID card.

You may also be eligible for other services such as Transportation, Enhanced Care Services, and Community Supports. Ask your Medi-Cal managed care plan if you are eligible.
