CMQCC, supported by DHCS, is leading a Learning Collaborative with Managed Care Plans (MCPs) to implement the updated Transitional Care Services policy for pregnant and postpartum members within the PHM Policy Guide. The TCS policy supports Medi-Cal members transitioning between care settings and back to their community. This transition includes the end of pregnancies, regardless of how or where that pregnancy ends. DHCS has updated the TCS policy requirements for pregnant and postpartum members to guide more effective care delivery of services and to align with the DHCS Birthing Care Pathway efforts.
In the TCS Learning Collaborative, CMQCC is supporting a group of MCPs throughout California to implement the DHCS TCS policy update, with attention towards community-oriented and health equity-focused interventions. Learnings from the Collaborative will be shared across all MCPs and will be made available below.
